Smiley Dental accepts dental insurance coverage from Altus Dental. Dental services are available to you at Smiley Dental if you are enrolled in the following plans: The Altus Plus Plan, The Altus Preferred Plan, or the Altus POS plan.

Eligibility

To determine whether you are eligible for Altus Dental insurance you must speak to your employer or sponsor. Typically, an individual plan covers only you. A family membership will cover you, your spouse, and any children up to the age of 21. As well, the family plan covers mentally or physically handicapped children over the age of 21 who are incapable of making their own living.

All of Altus’ dental insurance plans suggest visiting an in-network dentist. An in-network dentist is a dentist that is part of Altus’ dental providers. Smiley Dental is an in-network Altus dental practice that has accepted Altus’ insurance in hopes of giving you the best service at a more affordable price. The following plans that Smiley Dental accepts and Altus Dental offers are: The Altus Plus Plan, The Altus Preferred Plan, and the Altus POS plan. All of these plans just serve as a baseline and can be customized to best fit your needs.

The Altus Plus Plan

Smiley Dental accepts the Altus Plus Plan

The Altus Plus Plan is either an employer sponsored plan or can be signed up for individually and also as a family. The benefits of this plan are that the same coverage is given when visiting in-network dentists and out-of-network dentists.

Procedures that are covered by the Altus Plus Plan at Smiley Dental

The Altus Plus Plan follows the 100-80-50 percentage structure of dental care for in-network and out-of network dentists:

  • 100% of the cost of preventive care procedures such as periodic examinations and X-rays, cleanings and periodontal maintenance, sealants, space maintainers, and application of fluoride are covered.
  • 80% of the cost of basic procedures such as restorative fillings, endodontics,  periodontics, and oral surgery are covered.
  • 50% of the cost of major procedures such as implants, crowns and other cast restorations, and dentures and bridges are covered.

Calendar Year Costs in the Altus Plus Plan

  • Deductible  Typical deductibles are $50 dollars per person or $150 dollars per family. These are typical costs, these may vary depending on your plan and your provider.
  • Annual Maximum A typical annual maximum is $1500 per plan member for a policy year. However, this is subject to change depending on your employer and your specific plan

The Altus Preferred Plan is either an employer sponsored plan or can be signed up for individually and also as a family. Smiley Dental is considered to be an in-network dentist which allows for a more affordable experience due to a different coverage scheme.

Procedures that are covered by the Altus Preferred Plan at Smiley Dental

The Altus Preferred plan follows the 100-80-50 percentage structure of dental care for in-network dentists:

  • 100% of the cost of preventive care procedures such as periodic examinations and X-rays, cleanings and periodontal maintenance, sealants, space maintainers, and application of fluoride are covered.
  • 80% of the cost of basic procedures such as restorative fillings, endodontics,  periodontics, and oral surgery are covered.
  • 50% of the cost of major procedures such as implants, crowns and other cast restorations, and dentures and bridges are covered.

The Altus Preferred plan follows the 80-64-40 percentage structure of dental care for out-of-network dentists:

  • 80% of the cost of preventive care procedures such as periodic examinations and X-rays, cleanings and periodontal maintenance, sealants, space maintainers, and application of fluoride are covered.
  • 64% of the cost of basic procedures such as restorative fillings, endodontics,  periodontics, and oral surgery are covered.
  • 40% of the cost of major procedures such as implants, crowns and other cast restorations, and dentures and bridges are covered.
